An unannounced monitoring inspection was completed on 04/09/2019. Present during the inspection were 40 children, ages 3 months-5 years, in the direct care and supervision of eight staff members. The inspector reviewed 5 children's records, 5 staff records and 3 medications. The children were observed in the following activities: during lunch, during nap and participating in free choice activities. Infants were observed being fed and allowed to sleep on demand. Proper diapering procedures were observed as well. The inspector reviewed the requirement of the daily infant logs, the parent handbook, the emergency preparedness plans and the emergency supplies during the inspection. The inspector discussed subsidy updates with the staff. The inspector arrived for the inspection at 11:00 a.m. and departed at 4:00 p.m. If you have any questions, please contact Becky Forestier at 540-309-2835.
Standard #: 22VAC40-185-570-C Description: Based on document review and discussion with the staff, the facility failed to ensure that the record for each child on formula shall contain the brand of formula and the child's feeding schedule. Evidence: The document posted in the infant room listing each child's feeding requirements and feeding schedules did not list the brand of formula for Child #5 and Child #6. The posted document only indicated that Child # 5 and Child #6 were on "formula" and did not specify the brand of formula for the child. Plan of Correction: The chart was corrected during the inspection. The director will ensure in the future that the chart has the formula brand name on it.
