An unannounced monitoring inspector was conducted today 4/11/2019 . The inspection began at 7: 30 a.m and concluded at 11:30 a.m. There were twenty-two children in care with five direct care staff. During the inspection both buildings were viewed to include all four classrooms, diapering was observed, hand washing was observed, breakfast was observed, infant care was observed , nap time was observed , emergency supplies were viewed, and some policies and procedures were reviewed. Discussion with the director about: location of the violation notice on the lower building, making sure the formula is dated and labeled with the center's name, ensuring that all current staff have finger print eligibly letter.
Standard #: 22VAC40-185-160-C Description: Based on review of five staff's records, the center failed to ensure that all TB screening were completed every two years. Evidence: 1. Staff # 4 most recent TB screening was 4/4/2017. Plan of Correction: The director stated that the staff person has an appointment next week for a TB. The dates will be monitored.
Standard #: 22VAC40-185-510-J Description: Based on observation the center failed to ensure that all medication was locked. Evidence: 1. Observed in the infant room, an bottle of adult ibuprofen was stored in an child's # 2 bin and was unlocked. The medication was not labeled. Plan of Correction: The medication was removed and taken to the director's office to be given back to the parent at the end of the day. All parents will be remind that no adult medication or any medication is to be left at the center with out the center's knowledge.
Standard #: 22VAC40-185-520-C Description: Based on observation, the center failed to ensure that all ointments were labeled as required by the standard. Evidence: 1. Observed a jar of Vaseline for child # 5 that was used as a diaper ointment that was not labeled with the child's name. Plan of Correction: During the inspection the teacher labeled the jar and will make sure in the future that all ointments are labeled.
Standard #: 22VAC40-191-60-B Description: Based on review of five staff's records the center failed to ensure that all sworn statements were completed. Evidence: 1. Staff person # 1 did not have a sworn statement completed with the hire date of 9/18/2018 on the day of the inspection. Plan of Correction: The director will have the staff person to complete a new sworn state ASAP. All new hires will have a sworn statement prior to starting the position.
