Comments:
An unannounced mandated inspection was conducted today between 12:10pm and 3:00pm. There were 97 children present with 16 staff members, within the staffing guidelines. The children were observed waking from nap and beginning to eat snack. Snack consisted of Oreo cookies and apple juice. The physical plant, children's/staff files, emergency plans, menus, emergency supplies and evacuation drill logs were reviewed. Areas of non compliance were cited on the Violation Notice. Description: Based on children's record review, the center failed to maintain up to date records for all enrolled children. Evidence: Child #5's file did not have documentation of names of person authorized to pick up the child.

Plan of Correction: I will get the parents to fill out the form and put it in the child's file.
