Comments:
An unannounced renewal inspection was conducted today from noon - 3:30 pm. There were 61 children present with 10 staff. During this Spring Break week, I observed school agers busy with Legos, reading to each other in an open teepee; involved in role playing and playing outdoors. I also observed children listening to a story, talking about spring, and toddler movement activities in the infant-toddler class. Attractive variety of age appropriate materials available. Medications were well-organized and records were complete. If you have any questions, please contact me at brenda.kuhlman@dss.virginia.gov

Violations:
Standard #: 22VAC40-185-70-A
Description: Based on a review of records, one staff did not have all required items. Evidence: Staff # 1, hired 3/16, did not have references available for review. The references were obtained through a contractor.

Plan of Correction: I e-mailed the company and will have it soon. I should be able to access it to print it so got an error reference.

Standard #: 22VAC40-185-270-A
Description: Based on observation during today's inspection, there were some pieces of equipment that were found to not be maintained in a safe and operable condition. Evidence: Outdoors- There were four tricycles that required maintenance - two were missing a pedal, one had torn material covering the tire, and one had a protruding metal screw. A large gap was present in the concrete sidewalk directly outside the school age class, posing a tripping hazard. An area between the wire fence bottom and board surface created a gap that was accessible, posing an ankle entrapment. Exposed netting was observed where rolls of grass/sod had been placed, but the grass did not cover the majority of the area, leaving the netting as a tripping hazard. There was a gap between the fence and the building, approx 5 inches wide, and a broken area in the resilient material at the bottom of a slide.

Plan of Correction: I will contact Facilities and create a plan for them to come out asap.

Standard #: 22VAC40-185-280-B
Description: Based on observation during today's inspection, hazardous substances were observed to in an unlocked area. Evidence: The room where the washing machine/dryer is located was unlocked, and cleaners were observed inside.

Plan of Correction: Locked during inspection.
