A Subsidy Health and Safety inspection was conducted on 4/17/2019 to review supplemental health and safety requirements. The inspection began at 8 am and concluded at 1:30 pm. There were a total of 57 children present with nine staff members. During the inspection five children's, two agents, and six staff's records were viewed, three buildings were viewed, emergency supplies were viewed, infant care was observed, diapering was observed, food service was viewed, emergency supplies were viewed and polices and procedures were reviewed. The center does not administer any medication per policy and does not provide any transportation. A licensing inspection was also conducted this date; the two inspections combined cover all subsidy health and safety requirements. Only subsidy requirements that are not covered in licensing standards are reviewed during this supplemental inspection. Violations were found during the supplemental subsidy health and safety inspection requirements and are identified on the violation notice.
Standard #: 22VAC40-665-540-A Description: Based on review of six staff records, the center failed to ensure that all new hires had TB within the last 30 calendar day of the date of employment. Evidence: 1. Staff # 2 was hired on 3/20/2019 and the date of the TB test was 4/12/2019. 2. Staff # 3 was hired on 4/1/2019 and currently does not have a TB test on the day of the inspection. Plan of Correction: In the future all TB test will be completed according to the requirement.
Standard #: 22VAC40-665-580-F Description: Based on review of six staff's records, the center failed to ensure that all staff had first aid and CPR within 90 days of hire. Evidence: 1. Staff # 5 was hired on 11/15/2018 and on the day of the inspection did not have documentation of first aid and CPR. Plan of Correction: Administration will have staff # 5 to enroll in a class ASAP.
Standard #: 22VAC40-665-650-E Description: Based on review of policy and procedures the center failed to ensure that a written plan was developed that describes how the center will ensure that each group of children receives care by consistent staff or teach of staff members. Plan of Correction: Administration will develop the policy.
Standard #: 22VAC40-665-770-B-5 Description: Based on review of procedures, the center failed to ensure that continuity of operations to ensure that essential functions are maintained during a emergency must be included in the emergency response plan. Plan of Correction: Administration will develop the procedure.
