A Subsidy Health and Safety inspection was conducted on 4/4/19 to review supplemental health and safety requirements. Licensing Inspector arrived at 12:00 and departed at 1:30 PM to conduct an unannounced renewal inspection. At the time, there were six children (aged 12 months through four years in care with one staff member. Children were observed napping. The sample size was three staff and six children. A licensing inspection was also conducted this date; the two inspections combined cover all subsidy health and safety requirements. Only subsidy requirements that are not covered in licensing standards are reviewed during this supplemental inspection. Violations were found during the supplemental subsidy health and safety inspection requirements and are identified on the violation notice.

Standard #: 22VAC40-665-580-B Description: Based on review of three staff records, it was determined that the vendor did not ensure that all staff completed Virginia Preservice Training for Child Care Staff within 90 days of employment. Evidence: Staff #3, whom staff #1 verified had been working since December, did not have documentation of completing preservice training on file. Plan of Correction: Center management will ensure that staff #3 completes preservice training.
Standard #: 22VAC40-665-580-F Description: Based on review of three staff records, it was determined that the vendor did not ensure that all staff completed cardiopulmonary resuscitation (CPR) and first aid within 90 days of employment. Evidence: Staff #3, whom staff #1 verified had been working since December, did not have documentation of completing CPR and first aid on file. Plan of Correction: Center management will ensure that staff #3 completes CPR and first aid training.
