Technical assistance today included physical environment and record keeping.

An unannounced monitoring inspection was conducted today from 12 - 3:30 pm. There were 53 children present at the beginning of the inspection. All rooms were within ratio. I observed children playing with Legos and tabletop eating lunch; transition to nap and a smooth naptime. I also observed dramatic play, craft activities and afternoon snack. Standard #: 22VAC40-185-270-A Description: Based on observation during today's inspection, there were some areas that were not maintained in a safe condition. Evidence: Indoors: There was chipping paint accessible in two children's rooms. Outdoors: There were sharp edges at child height in the metal edges under the siding. SplinterS were observed in one of the wooden boards, a small amount of peeling paint was accessible on the large slide and uneven surfacing in the corner of the basketball area. Plan of Correction: We will have the painting touched up. For the edges, electrical tape will be placed to cover sharp areas. For splintering wood, it will be sanded. Mulch will be swept so it is even.
Standard #: 22VAC40-185-340-F Description: Based on observation during today's inspection, children under age ten were not consistently within hearing distance when using the bathroom. Evidence: With one staff present at after-lunch transition time, children were observed today to be sent individually to the bathroom unescorted. Plan of Correction: I have already talked to the teacher about lining up the children in the hallway with a book so they are within hearing.
Standard #: 22VAC40-185-510-A Description: Based on a review of medications on site today, some items did not meet standards. Evidence: Of three medications reviewed, Child # 2 had an authorization that did not give this school permission to administer the medication. Child # 3 was prescribed Benedryl, but no dosage was written. Plan of Correction: I will inform the parents. We will send a new authorization form home.
Standard #: 22VAC40-185-550-M Description: Based on a review of records today, not all injury reports included all required information. Evidence: Reports from the following days did not have complete information related to the time and method that the parent was notified: 3/1, 3/28; 4/2. Plan of Correction: I will re-train staff in writing accident reports.
Standard #: 22VAC40-185-560-F Description: Based on observation, staff confirmation and a review of records today, today's lunch did not meet all USDA minimum requirements for preschool and schoolaged lunch. Evidence: Children were observed eating spaghetti with a meatless spaghetti sauce, green beans and pears. It had been sprinkled parmesan cheese, but it wasn't equal to the required amount for the protein component (2 ounces per schoolager). Plan of Correction: I will inform the staff of the requirement.
Standard #: 22VAC40-191-40-D-1-B Description: Based on a review of records today, there was no documentation of an inititial Search of the Central Registry for one staff. Evidence: Staff # 1, with a hire date of 2/21/19, did not have documentation that a response had been received or that the record was checked on within 30 days as required. Plan of Correction: The employee started March 11 in Woodbridge and transferred from that branch of the company to this site on April 1. The director at the Woodbridge center contacted OBI today to follow up on it.
