We discussed the requirements for staff records. The risk assessments for violations were provided.
Thank you for your assistance during the unannounced monitoring inspection conducted from 2:40 PM to 5:45 PM. Today, there were twenty-seven children, ages five through ten years, with five staff. It was reported that thirteen children were supervised by two staff before school. I viewed program activities, daily routines, staffing, supervision, interactions with children, equipment and supplies, posted information, injury records, emergency plan, emergency supplies and drill records, five records for children, five staff records, staff qualifications, staff training, snack, nutrition, agency inspection reports, indoor and outdoor safety. The children have received regular opportunities for homework assistance. Today, the children enjoyed active play outside during the warm sunny day. Standard #: 22VAC40-185-160-C Description: Based on a review of five records for staff, and an interview with administrative staff, the center failed to obtain an updated tuberculosis screening and statement by two years from the date of the most recent statement for one staff member. Evidence: The TB screening statement completed on October 3, 2016 for staff member 3 was not updated until November 21, 2018. Plan of Correction: Administrative staff will use a tracking system for record dates and arrange for staff to submit updated TB statements by two years from the date of the last screening results.
Standard #: 22VAC40-185-70-A Description: Based on a review of five records, and an interview with administrative staff, the center failed to obtain written information for one staff member to demonstrate that the individual possesses the education required by the job position. Evidence: A copy of the high school diploma or transcripts could not be located in the record of staff member 2. Staff member 2 was promoted to a program leader permission on August 25, 2018. Plan of Correction: An administrator arranged for staff member 2 to request high school transcripts which will be provided for the record. Administrative staff will make sure written information to demonstrate the education qualifications for the job position is obtained at the time of hire or promotion.
