Standard #: 22VAC40-185-70-A Description: Based on record review, the center did not obtain two references for an employee before the first day of employment. Evidence: Staff # 1's record had only one reference check on file. There was no documentation of a second reference check. Plan of Correction: We will follow up with our corporate office to get documentation of the second reference check.
Standard #: 22VAC40-185-510-C Description: Based on records review, written parental authorization for medication administration was expired for children's medication. Evidence: 1. The written parental authorization for 2 of 2 emergency medications for Child # 1 expired on 4/6/19. 2. The written parental authorization for 1 of 2 emergency medications for Child # 2 expired in January 2019. The written parental authorization for 2 of 2 medication for Child # 2 expired on 12/28/18. Plan of Correction: We will ask the parents to update the forms.
Standard #: 63.2(17)-1720.1-B-3 Description: Based on record review, the center did not obtain an out of state central registry finding for an employee who has resided outside of the state of Virginia within the last five years. Evidence: 1. Staff # 1 was hired on 2/20/19. Staff # 1's records indicated that she has resided outside of Virginia within the last five years. 2. Staff # 2 stated that Staff # 1's out of state central registry was mailed but the results have not yet returned. Plan of Correction: We will conduct a follow up on the results.
