Thank you for your assistance during this unannounced complaint inspection conducted on April 19, 2019 from 2:00 p.m. through 3:00 p.m. This on-site visit was initiated as a result of two complaints that were received by the Valley Licensing Office on April 15, 2019 and April 18, 2019. The complaints were in reference to the facility not following policies and procedures for identifying parent/guardian during pick-up as well as supervision. Three staff were interviewed and one child's record was reviewed and well as the sign in and sign out sheet. The information gathered during this inspection does not support the allegation, therefore, the complaint is "not valid."
