Standard #: 22VAC40-185-140-A Description: Based on a review of eight children's records and interview on 04/09/2019, the center did not ensure that one child had a physical examination by or under the direction of a physician before the child's attendance; or within one month after attendance. EVIDENCE: 1) The record for Child #6 (DOE: 09/04/2018) did not contain a physical examination. 2) During interview, a member of management confirmed the physical examination report for Child #6 had not been received to date. Plan of Correction: Per the Center: We are working with the parents in acquiring the missing physical.

Standard #: 22VAC40-185-280-B Description: Based on observation at the center on 04/09/2019, the center did not ensure to keep all hazardous substances such as cleaning materials in a locked place using a safe locking method that prevents access by children. Evidence: 1) The licensing inspector observed a bottle of hand sanitizer on a counter top in the Preschool room. Soiled diapers/pull-ups should be disposed of in a leak proof, covered disposable system that is either foot operated or used in such a way that the staff member's hand nor the soiled diaper touches the exterior surface of the storage system. Plan of Correction: Per the Center: We have placed a new trash can with a foot operated release in the Discovery Preshool restroom.
