Technical Assistance:
Technical assistance today included acquiring a pill-cutter for one child's medication; and physical environment.

An unannounced monitoring inspection took place today from 10:30 am - 1:30 pm. There were 47 children present; with schoolagers arriving later. All rooms were within ratio. I observed children singing, dancing and listening to a children's entertainer who brought a guitar, train whistle and other props. Description: Based on observation during today's inspection, the following areas were found to not be maintained in a clean/safe/operable condition. Evidence: Indoors: The foosball table in the schoolage room had very shaky legs held together with cellophane tape. One of the children's toilets was covered with plastic. Staff reported it didn't always work correctly. Outdoors: Tripping hazards were observed in the concrete in several areas: directly outside the door to the classroom, there was broken concrete. Nearby, there was a triangular-shaped broken concrete area (approx. 9 x9 x 10). In the area near the smaller playground, the sidewalk ends suddenly, leaving an uneven surface where children play. Other tripping hazards included exposed netting from where grass and sod had been present; protruding wood (four small pieces) from an older structure no longer there that make the sidewalk uneven; and small wooden exposed posts in the field/grassy area where children play ball. There was also a broken plastic dryer vent accessible at child height; peeling brown paint accessible on the metal fence at child height and some bolts on the roof of the smaller play structure which were beginning to protrude.

Plan of Correction: Indoors: The foosball table was removed today. A work order will be put in for the toilet. Outdoors: Facilities is scheduled for the first week of May to resod and seed the grass. A work order will be put in to secure the bolts, remove the brown paint from the fence, and install a stronger dryer vent that is appropriate at child height or place it elsewhere. A work order will be put in again to remove tripping hazards by replacing or repairing concrete areas and removing the wooden obstacles.

Description: Based on a review of medication on site today, there was one child's medication/treatment plan which did not meet the standards. Evidence: The emergency allergy medication expired 1/19, and the antihistamine documented as an action to take on the Allergy Action Plan had not been brought in since it was authorized 12/18.

Plan of Correction: The parent was contacted during the inspection.
