Technical Assistance:
This initial inspection conducted on April 23, 2019 is for the new facility, Building Kidz. Currently, the previous owners are managing the facility and care of children while the new license and application are being processed.

Comments:
An announced initial inspection was conducted today from 10:00am-12:45pm. There were 40 children (infants-5yrs.) directly supervised by 8 staff. The physical plant, 6 staff records, 1 child?s records, medications, evacuation drills, emergency supplies, and policies were inspected. Children were observed participating in group play, and reading books. There was a supply of materials for the children. There was an adequate number of staff with current certification in MAT, CPR and First Aid, as well as DHO training. Staff-to-child ratios were in compliance. Areas of non-compliance are identified in the Violation Notice. Violations:
Standard #: 22VAC40-185-270-A
Description: Based on observation, areas and equipment were not maintained in a safe condition. Evidence: 1) On the lower level, there is a small corner area the paint is bubbling and in one section the paint has started to chip. 2) In the classroom on the lower level, there is a hole in the wall exposing a pvc pipe which could cause a potential finger entrapment. Underneath that area, the baseboard is coming off the wall and there is a small hole exposing dry wall and paint.

Plan of Correction: Currently, the lower level is not being utilized. We are aware of these areas and have our maintenance person scheduled to make repairs.

Standard #: 22VAC40-185-540-E
Description: Based on observation, the center did not maintain all required nonmedical emergency supplies. Evidence: The center does not have batteries for the radios available in the center.

Plan of Correction: We will purchase batteries this afternoon.

Standard #: 22VAC40-185-550-C
Description: Based on observation, the center does not have emergency evacuation and shelter-in-place procedures/maps posted in a location conspicuous to staff and children on each floor of each building. Evidence: The lower level has a map but does not have procedures and exits mapped.

Plan of Correction: Currently the lower level is not being utilized. We will place maps and procedures on lower level.
