Comments:
An unannounced monitoring inspection was conducted today from 10:30am-3:00pm. There were 140 children (infants-5yrs.) supervised by 30 direct care staff. The physical plant, 4 staff records, 10 children?s records, medications, evacuation drills, injury reports, emergency supplies, and policies were inspected. Children were observed going on an outdoor buggy ride, painting, playing with a felt board, sensory table, in home living, and having sign-language classes. Toileting and hand-washing procedures were also observed. There was an abundance of books, toys, and materials, which were found to be developmentally appropriate for the children in care at the time of inspection. Areas of non-compliance are identified in the Violation Notice. Standard #: 22VAC40-185-60-A
Description: Based on a review of children's records, 2 of 10 children's records did not contain documentation of all required information. Evidence: 1. Child #8's (start date: 9/10/2018) record did not contain documentation of child updates and confirmation of up-to-date information in the child's record as required by 22 VAC 40-185-420 E 3. The most recent documented parental review of emergency contact information was dated 4/5/2018. 2. Child #9's (start date: 9/10/2018) record did not contain documentation of previous child day care and schools attended by the child.

Plan of Correction: Immediate and ongoing, we will update the forms every September.

Standard #: 22VAC40-185-280-B
Description: Based on classroom observations, 3 of 10 classrooms did not ensure that hazardous substances were kept in a locked place using a safe locking method that prevents access by children. Evidence: 1. In the Apples classroom (children ages 1 year old), a bleach solution spray bottle was found inside of the children's bathroom on a shelf. Two bleach spray bottles were found in the naptime area/room on a shelf above the diaper changing table. 2. In the Bears classroom (children ages 2 years old), a container of clorox wipes were found in an unlocked cabinet under the diaper changing table. 3. In the Goldfish classroom (children ages 4 years old), two containers of shaving cream were found in an unlocked cabinet above the counter.

Plan of Correction: We will immediately be moving the spray bottles into locked cabinets, and will continue to ensure that they remain locked moving forward.

Standard #: 22VAC40-185-560-G
Description: Based on classroom observation, 2 of 10 classrooms contained food containers brought from home that were not sealed and clearly dated and labeled in a way that identifies the owner. Evidence: 1. In the 3's Preschool classroom (children ages 3 years old), 3 of 6 children's lunch bags checked, were not labeled with the current date. 2. In the Starfish classroom (children ages 4 years old), 2 of 6 children's lunch bags checked were not labeled with the current date.

Plan of Correction: Dates will be added today, and all children's lunch bags will be labeled with names and dates ongoing.
