Standard #: 22VAC40-185-270-A Description: Based on observation the center did not ensure all areas and equipment of the center, inside and outside, were maintained in a safe condition. Evidence: 1. In the 2.5-3 year-old room and the hallway there was visible rust on the floor. An orange/brown substance came off on a white tissue when rubbed. 2. There was peeling paint in the cubbies in the preschool-A and hallway cubbies. 3. The wooden window sill frames in the pre-k classroom had chipped wood, that was splintered. 4. On the playground, the climbing apparatus had rusted bolts. The bolts also were stacked on top of eachother with the screws outside the bolts, causing a potential strangulation and protrusion hazard. Plan of Correction: The following corrective action was provided during the exit interview: The rust will be removed, cubbies painted and window sills will be replaced or sanded. The piece of equipment on the playground will be removed.
