Technical Assistance:
Licensing inspector discussed emergency contact requirements for staff.

Comments:
An unannounced renewal inspection was conducted today from approximately 10:30am-1:50pm. There were 137 children directly supervised by 13 staff. A sample of 10 children?s, 6 staff records were reviewed. The children were observed playing on the playground, eating lunch, reading books, singing and dancing. The center will begin spring break next week. One new staff has been hired since the last inspection. Areas of non-compliance are identified in this report. Standard #: 22VAC40-185-40-H
Description: Based on interview and observation, the sponsor did not maintain public liability insurance for bodily harm for each center site. Evidence: The liability insurance record available did not cover bodily harm.

Plan of Correction: Head Start Administrator submitted requested document from Fairfax County Government to be available onsite fro reviews/inspections on April11 2019.

Standard #: 22VAC40-185-40-J
Description: Based on observation, the center did not have injury prevention procedures. Evidence: The center did not have injury prevention procedures that should be updated at least annually.

Plan of Correction: Head Start Coordinator will create an injury prevention procedure with management to implement September 2019.

Standard #: 22VAC40-185-60-A
Description: Based on record review, children's files were incomplete. Evidence: 6 of 6 children's files reviewed were missing required information. 1) Children A-H were missing first date of enrollment and school previously attended. 2) Child B was missing the father's home and employment information. 3) Child D was missing parents employment information. 4) Child E was missing mother's employer phone number. 5) Child G was missing mother's employer phone number.

Plan of Correction: Children's files will be updated to reflect the required information next school year.

Standard #: 22VAC40-185-280-B
Description: Based on observation, hazardous substances were not kept in a locked place. Evidence: In Room #4, a cabinet which held cleaning supplies was found unlocked.

Plan of Correction: Corrected during the inspection.
