Comments:

An unannounced complaint inspection was completed on 04/16/2019 from 10:20 A.M.- 11:30 A.M. The inspection was in response to a complaint that was received in the Valley Licensing Office on 04/10/2019 regarding to hazardous materials, and age and stage appropriate materials. Interviews with four staff were conducted, four staff records reviewed, and one classroom observation was observed. The information gathered during the investigation does support the allegation , so the allegation is determined to be "valid". The exceptions to compliance are noted on the violation notice. Description:	Based on observation and five staff interviews, the center failed to ensure that all medications were kept in a location that was not accessible to children. Evidence: 1. On 04/16/2019, in Classroom A, Staff #1's black purse was hanging on a hook in the closet. The closet does not have a locking mechanism, and the hook is accessible to children. 2. The purse contained one bottle of over-the-counter medication and three pill bottles of prescription medications. 3. Staff #1 reported that the closet door does not have a locking mechanism. 4. Staff #5 confirmed that the purse was stored in the closet with no locking mechanism.
Plan of Correction:	A medication safety procedure has been created and all staff will need to review, sign and acknowledge the new procedure. All personal pocket books must be kept in a locked cabinet or in the staff's car.

Description:	Based on interviews with five staff, the center did not ensure that age and stage appropriate materials were used with children. Evidence: Staff # 1 stated that that most days for the past two months at breakfast time, in Classroom A, she would retrieve her personal purse from the hook in the unlocked closet. She would remove one bottle of over-the-counter medication and three pill bottles containing prescription medication from her purse. Staff #1 would sit at the table with a group of four year old children and take each pill out of the container. Staff #1 would place the medication in her hand and the children would count the medications. They would discuss the shape and color of the medication. Staff #1 would then place the medications in her mouth and swallow.
Plan of Correction:	According to the new medication safety procedure no medications can be consumed in the classroom. If staff need to take any kind of medication they will have to call to be relieved from their duties in the classroom so medications may be take in private in an area away from all children.
