Comments:
An unannounced monitoring inspection was done today 4/30/2019. The inspector arrived at 6:30 A.M. and departed at 8 am. There were 28 children present today with four staff supervising. The inspector reviewed five children's and five staff's records during the inspection. Observed the children during: arrival, and free play.

Violations:
Standard #: 22VAC40-185-60-A
Description: Based on review of five children's records, the center failed to ensure that all records were complete. Evidence: 1. The inspector found that child # 1 had the father for one of the emergency contacts. Emergency contact are for when the parents can not be reached. 2. Child # 4 was missing the address for one of the emergency contacts.

Plan of Correction: The program director will have the parent to complete the missing information and will monitor all records.
