Description: Based on review of seven children's files and staff interview, the facility failed to have a written agreement between the parent and the center in each child's file by the first day of attendance to include: a statement that the parent will inform the center within 24 hours or the next business day after his child or any member of the immediate household has developed any reportable communicable disease, as defined by the State Board of Health, except for life threatening diseases which must be reported immediately.
