Comments:
The licensing inspector conducted an unannounced monitoring inspection Friday, April 26, 2019 from10:00am to 1:30pm. The children were observed in "open time". During open time the children are self directed and choose which learning classroom they would like to work in. They were also observed playing outside on the playground. All classrooms and playgrounds were inspected today. Ten children?s records and ten employee records were reviewed during this inspection. Medication is being administered and was reviewed. The center's first aid kit and emergency supplies were inspected. Last emergency drill: 03/26/2019 Shelter-in-place drills: 03/19/2019 Health Inspection: 03/04/2019 Fire Inspection: 03/18/2019 Today, the following child to staff ratios were observed: Pink Room -11:2 Purple Room -11:2 Yellow Room -11:2 Red Room-12:2 Blue Room -11:2 Rainbow Room -13:2 Orange Room-11:2 Polka Dot Room-11:2 Green Room -10:1 The violations from the previous inspections were checked for corrections. Description: Based on a review of children's records on 04/30/2019, the center did not ensure that documentation that each child has received the immunizations required by the State Board of Health was obtained before the child can attend the center. EVIDENCE: The immunizations in the record of Child #3, who started at the center on 09/05/2017, were dated 09/12/2017.

Description: Based on a review of staff records on 04/26/2019, the center did not ensure that staff members shall obtain and submit the results of a follow-up tuberculosis(tb) screening every 2 years. EVIDENCE: The most recent tb screening in the record of Staff #10 is dated 12/06/2016.

Description: Based on a review of children's records on 04/30/2019, the center did not ensure that one three of ten records reviewed contained the required information. EVIDENCE: 1. The first emergency contact listed in the record of Child #4 did not have an address listed. 2. The records of Child #3 and Child #8 did not contain the number for the place of employment listed for the child's father.
