Comments:
The licensing inspectors conducted an unannounced renewal inspection on Tuesday, April 30, 2019 from approximately 3:00 pm to 5:00 pm to determine the center's compliance with licensing standards. The census for today's inspection consisted of 24 children and five staff including administrative staff to assist as needed. The licensing inspectors observed the children in care completing a variety of activities to include eating snack, completing homework, interacting with peers and staff, engaged in activities to include puzzles and coloring activities, and taking bathroom breaks. The program's snack for today consisted of trail mix and apple juice. The facility's playground area, bathrooms areas, and classrooms/materials were inspected. There were six children?s records and three staff's record reviewed during this inspection. The required posting were inspected. The Department has not yet received a complete renewal application and annual fee from the center. The Department must receive a complete renewal application and fee prior to expiration of the license on 6/22/19 or the Department cannot renew the license. Standard #: 22VAC40-185-60-A
Description: Based on review of children's record, the center did not ensure to obtain all required information. Evidence: During the inspection on April 30, 2019, in Child#5's and Child#6's records (start dates: 3/11/19) the one of the designated person's name, address, and contact number was provided but this person cannot be a parent.

Plan of Correction: The staff obtained required information for the record before the inspection was complete. The program will review children's record to ensure all required information is obtained.

Standard #: 22VAC40-185-550-D
Description: Based on review of emergency drills log and interview with staff, the center did not ensure to practice monthly drills as required. Evidence: During the inspection on April 30, 2019, the last documented monthly drill was dated for 10/18 for the year 2018. Interview with staff confirmed the drills for the month of November and December 2018 were not available.

Plan of Correction: The center will ensure to practice monthly fire drills and record in the emergency drills log as required.
