Comments:
A Subsidy Health and Safety inspection was conducted today from 2:30pm-4:45pm to review supplemental health and safety requirements. 60 children were observed in the care of 12 staff. A sample of 10 children's records, 3 staff records, 10 injury reports, emergency drills and other documents were reviewed. Children were observed during transitions from naptime, toileting, handwashing, snack and outdoor play. A licensing inspection was also conducted this date; the two inspections combined cover all subsidy health and safety requirements. Only subsidy requirements that are not covered in licensing standards are reviewed during this supplemental inspection. Violations were found during the supplemental subsidy health and safety inspection requirements and are identified on the violation notice. Description: Based on a sample review of records, the records for 2 children with diagnosed food allergies did not include documentation of a written allergy care plan. Evidence: 1. The record for Child #2 with a diagnosed food allergy to papaya did not include a written allergy care plan. 2. The record for Child # 9 with a diagnosed food allergy to treenuts did not include a written allergy care plan.

Plan of Correction: I will make sure the medications and allergy care plans are provided before enrollment.
