Arrived 9:20am Departed 12: pm A Subsidy Health and Safety inspection was conducted on 05/03/2019 to review supplemental health and safety requirements. A licensing inspection was also conducted this date; the two inspections combined cover all subsidy health and safety requirements. Only subsidy requirements that are not covered in licensing standards are reviewed during this supplemental inspection. Violations were found during the supplemental subsidy health and safety inspection requirements and are listed on the violation notice. Thank you for your cooperation. Standard #: 22VAC40-665-540-A Description: Based on the review of seven staff files, the facility failed to ensure that all staff have a TB test or screening before coming into contact with children. Evidence: 1. Staff #6 has not completed a TB test or screening. (hire date 8-3-18) Plan of Correction: The director will ensure that staff #6 has a TB test or screening.
Standard #: 22VAC40-665-580-B Description: Based on a review of seven staff files and an interview with the director, the facility failed to ensure that all staff have completed the VA preservice training. Evidence: 1. Staff #6 has not completed the required preservice training. Plan of Correction: The director will ensure that staff #6 completes the training.
