Reminder: Two shelter-in-place drills need to be practiced annually.

Comments:
The licensing inspector conduced an unannounced renewal inspection from 9:10am - 1:00pm. There were twenty eight preschool children in care with three staff supervising. The children were observed in group activities, listening to a story, having morning snacks and at circle time. Seven staff's records, three board officer's records, and five children's records were reviewed. Violations from the previous inspection were also reviewed and two were repeated. Violations:
Standard #: 22VAC40-185-270-A
Description: Based on observation, the center failed to maintain the outside play area in a safe condition. Evidence: 1. The inspector observed the playground. There is a wood playhouse and a wood car located on the preschool playground. Both structures have peeling paint. Also, there is felt protruding through the mulch in front of and behind the swings creating tripping hazards.

Plan of Correction: The wood playhouse and car will be repainted by the end of this month. Additional mulch will be raked into the areas in front of and behind the swings to cover the felt.

Standard #: 22VAC40-185-280-B
Description: Based on observation, the center failed to have all hazardous substances stored in a locked place. Evidence: 1. The inspector observed the bathrooms. In the bathroom (located adjacent to the Jr. Kindergarten classroom) there is a wall cabinet that contained Clorox wipes, Clorox Kitchen Cleaner and Comet. There was a lock on the cabinet door, but it was not closed.

Plan of Correction: The teacher locked the cabinet during the inspection. Staff will be reminded to monitor to ensure the cabinets remain locked.

Standard #: 22VAC40-185-550-D
Description: Based on review of recorded drills, the center failed to document monthly evacuation drills. Evidence: 1. The inspector reviewed the documentation of evacuation drills. There were no drills documented for the months of January and April 2019.

Plan of Correction: The center will practice two additional drills to make up for the missing drills.

Standard #: 22VAC40-191-40-D-1-A
Description: Based on record reviews, the center failed to obtain background checks within the required time period. Evidence: 1. The inspector reviewed three board officer's records. Two of three records reviewed (board officer's records #2 and #3) did not contain fingerprint based criminal history background checks. These checks are required before a person begins serving on the board.

Plan of Correction: Board officers #2 and #3 will schedule appointments to obtain their fingerprint based background checks this week. The licensing office will be notified when the center has obtained the results of the checks.
