An unannounced Monitoring Inspection was conducted on May 6, 2019 from approximately 9:25 am until 12:30 pm. Upon entrance there were 6 children and 3 staff present with children. Additional staff arrived during the inspection. The record review sample consisted of 5 children's records and 2 staff records. Children were observed singing, reading books, eating snack and playing with blocks and kitchen toys. The center is not administering medications. Areas of non-compliance were noted on the violation notice. The results of the inspection were reviewed and verified with the center director. Standard #: 22VAC40-185-270-A
Description: Based on observation and staff interviews, the licensee did not ensure all areas and equipment of the center, inside and outside, shall be maintained in a clean, safe and operable condition. Unsafe conditions shall include, but not be limited to, splintered, cracked or otherwise deteriorating wood; chipped or peeling paint; visible cracks, bending or warping, rusting or breakage of any equipment; head entrapment hazards; and protruding nails, bolts or other components that could entangle clothing or snag skin. Evidence: During the inspection on May 6, 2019, the window air conditioner unit in the schoolage room was observed and there were large amounts of dirt/dust observed in and around the unit, that when touched moves easily and comes off on your hand. The air conditioning unit was observed when it was turned on and dust/dirt was observed blowing out of the air conditioning unit . There was tape around the frame of the unit and that was peeling away from unit and has dirt/dust attached. The air conditioning unit is accessible to the children.

Plan of Correction: The center director will clean the dust and remove the tape around the unit by the end of the week.

Description: Based on documental review and staff interviews, the licensee did not ensure the center shall prepare a document containing all the required emergency documents. Evidence: During the inspection on May 6, 2019 the center's vehicle was inspected and the center director confirmed there was not a document on the van that contained local emergency contact information, potential shelters, hospitals, evacuation routes, etc., that pertain to each site frequently visited or of routes frequently driven by center staff for center business (such as field trips, pickup/drop off of children to or from schools, etc.). This document must be kept in vehicles that centers use to transport children to and from the center.

Plan of Correction: The center director will write the information and put it in the van today.

Description: Based on documentation review and staff interviews, the licensee did not ensure all required written documentation was maintained in the center 's vehicle. Evidence: During the inspection on May 6, 2019 the center's vehicle did not contain the center's address, and the number of 911 or local dial number for police, fire and emergency medical services and the number of the regional poison control center .

Plan of Correction: The center director will write the information and put it in the van today.
