An unannounced renewal inspection was completed on 05/07/2019 from 12:30 P.M. until 4:30 P.M. At the time of the inspection 37 children were in care with nine staff and two administrators present. Five children's records, seven staff records, two board members background checks, required posted information, evacuation drills, emergency plan, five injury reports, injury prevention plan, first aid kits and supplies, and outside play area were reviewed. Children were observed during diapering, handwashing, naptime, free choice activities, Mother Day art activities, snack time, outside play time, and departure. Standard #: 22VAC40-185-60-A Description: Based on review of children files, the center failed to ensure that all required information was in a child's file. Evidence: 1. Child #3's file was missing an emergency address for emergency contact number two. 2. Staff verified that the file was missing an address for emergency contact number two. Plan of Correction: Administration will contact parents at pick-up to obtain the address. Administrative staff will continue to audit children files to make sure all required information in in the file.
