An unannounced monitoring inspection was conducted from 7:10 - 8:10 am. There were 13 children upon arrival, ranging from six years to 11 years of age, present with three staff supervising. The children were observed participating in a variety of activities including reading, playing with army men, singing and dancing, and playing a variety of games. Required information was posted. Five children's files were reviewed. Five medications and authorization forms were reviewed. Two staff are Medication Administration Training (MAT) certified. The last emergency escape drill was conducted 4/23/19, and the last shelter-in-place drill was conducted 4/29/19. Standard #: 22VAC40-185-130-A Description: Based on a review of five children's files, the facility did not obtain documentation that each child has received required immunizations prior to the child?s first attendance. Evidence: The file for child #1 (date of first attendance: 1/3/19) did not have documentation of immunizations. Plan of Correction: Will ask the parent to provide the missing documentation.
Standard #: 22VAC40-185-140-A Description: Based on a review of five children's files, the facility did not obtain documentation that each child has received a physical examination before the child's first attendance or within one month after attendance. Evidence: The file for Child #1 (date of first attendance: 1/3/19) did not have documentation of a physical examination. Plan of Correction: Will ask the parent to provide the missing documentation.
Standard #: 22VAC40-185-510-G Description: Based on a review of five medications, the facility failed to have written authorization from the parent to administer the medication as required. Evidence: Child #2 did not have written parental authorization for the YMCA to administer three medications. Plan of Correction: Will ask the parent to provide the missing documentation.
