5/3/2019 Arrived 9:20am Departed 12:15 pm An unannounced monitoring inspection was conducted today. Children were playing outdoors, writing, singing, reading stories and eating lunch. Medications, previous violations, seven staff records, and six children's records were reviewed. There were 52 children in care with nine teaching staff, one administrative staff member, and one kitchen staff person. Thank you for your cooperation. Standard #: 22VAC40-185-270-A
Description: Based on a tour of the building and playgrounds, the facility failed to ensure that all areas are safe and operable. Evidence: 1. On the playground facing the interstate there is a basketball goal. The pole, and the screws that attach the pole to the base, are rusting. The pole moves back and forth. It presents a tipping hazard. 2. On the wooden seats of the red tricycle go round the paint is worn off and there is one bolt missing on one seat making the seat wobble back and forth. 3. In the K-4 room, the K-3 room and toddler room there are large portable TVs on top of cabinets. They present tipping hazards.

Plan of Correction: Will remove basketball goal from playground. 5/7/19 Sand and paint seats on tricycle go round and replace bolt. 5-17-19 Replace TV in K-3 and toddler room 5-10-19 Remove TV in K-4 room. 5-7-19

Standard #: 22VAC40-185-330-B
Description: Based on a tour of the playgrounds, the facility failed to ensure that there is six inches of mulch under all climbing equipment. Evidence: 1. On the large playground, there is less than one inch of resilient surface material at the end of the yellow slides. 2. On the large playground, there is less than two inches of resilient surface material under the blue dome climber. 3. On the toddler playground, there is hard packed dirt at the base of the slide.

Plan of Correction: Will order mulch to spread around slides and climbing equipment.

Description: Based on the review of six staff files, the facility failed to ensure that all staff meet the qualifications for their positions. Evidence: 1. There is no documentation in the file for staff #6 that she meets the qualifications for program leader.

Plan of Correction: Will ensure that staff gets the training needed to meet qualification for their position.

Standard #: 22VAC40-185-500-A
Description: Based on observation, the facility failed to ensure that all staff follow hand washing procedures. Evidence: 1. Staff #7 did not wash her hands before diapering a child.

Plan of Correction: Remind all staff to wash hands before and after diapering a child.
