Comments:
The Licensing Inspector conducted a joint investigation with a local agency in response to a complaint that was received by the licensing office on May 2, 2019. The investigation regarding supervision in the Toddler classroom was initiated on May 2, 2019 from 10:10am to 12:30pm. Documents and the center's video recordings were reviewed and interviews were conducted with staff. The information gathered during this investigation does support the allegations, so the complaint is determined to be valid. See the Violation Notice for findings. Please complete the columns for "Plan of Correction" and "Date to be Corrected" for each violation cited on the violation notice, and then return a signed and dated copy to the licensing office within 5 business days of receipt. Description: Based on record review, the center failed to obtain documentation of a negative tuberculosis screening for each staff member no later than 21 days after employment. Evidence: 1. Staff A was hired on 2/26/2019 and the center did not have documentation of a negative tuberculosis screening for the staff member.

Plan of Correction: Current TB Test is available for Staff A.

Description: Based on multiple staff interviews, review of the center's video recordings and documentation reviewed, while supervising children , staff did not always ensure their care, protection and guidance. Evidence: 1. On April 30, 2019, at approximately 9:43am Child #1(21 months) was "tackled to the ground" by Child #2 and was bit three separate times as described in the accident report and by Staff B. The child's back and right side had bite marks. Staff C observed a broken skin and teeth marks. 2. When the accident occurred, Staff A was sitting on the same carpet as Child #1 and Child #2. Staff A was facing the two children and did not get up to intervene or stop Child #2 from biting Child #1. Staff A was aware that Child #2 had bitten another child on April 29, 2019 and stated she reviews all incident and accident reports in the classroom to make sure they are following proper protocol and remembers reading the incident report from April 29, 2019. The April 29, 2019 report stated that Child #2 had bitten another child and that Child #2 will be monitored. 3. Staff B (who was engaged with another group of children with her back to Child #1 and Child #2) heard Child #1 scream and separated the children. Staff B stated that Child #1 was "pretty upset and crying."

Plan of Correction: The staff was retrained in appropriate supervision and responsiveness while in the classroom The classroom team was also retrained in appropriate supervision. A mandatory professional development training was scheduled for June 1st, 2019 for all staff members to provide tools and techniques for effective discipline, redirection, Teacher-children interaction & supervision.
