Comments:
An unannounced monitoring inspection was conducted on 5/10/19 from 11:10am until 1:20pm. At the time of entrance, there were 14 children in care with 4 staff present. Additionally, two children arrived to the center from school during the course of the inspection. Children were observed eating lunch, playing educational games on the computer, engaging in free play and resting quietly during nap time. Medication, first aid and emergency supplies, documentation of emergency practice drills, children's injury reports and center postings were reviewed. Three violations in the areas of administration and special care provisions and emergencies are identified on the violation notice and were discussed with the center owner during the exit interview.

Violations:
Standard #: 22VAC40-185-140-A
Description: Based on record review and interview, the center did not ensure that each child shall have a physical examination by or under the direction of a physician before the child's attendance or within one month after attendance. Evidence: 1. The record for child 1 does not contain documentation of a physical examination. Child 1 has a date of enrollment of 3/5/19 and was in care during the inspection. 2. The center owner confirmed that the record for child 1 is lacking documentation of a physical examination.

Plan of Correction: The center responded with the following: This was a paperwork oversight. Documentation of child 1's physical will be requested from the parent today.

Standard #: 22VAC40-185-160-C
Description: Based on record review and interview, the center did not ensure that staff shall obtain and submit the results of a follow-up tuberculosis screening at least every two years from the date of the first initial screening or testing. Evidence: 1. The most recent tuberculosis screening for staff 1 is dated 1/30/17. An updated tuberculosis screening for staff 1 was due in January 2019. 2. Staff 1 confirmed that she had not obtained an updated tuberculosis screening.

Plan of Correction: The center responded with the following: Staff 1 will obtain and submit an updated TB screening as soon as possible.

Standard #: 22VAC40-185-510-C
Description: Based on a review of medication, the center did not ensure that procedures for administering medication shall include methods to prevent use of outdated medication. Evidence: 1. There was an Epipen located in the center's medication lock box with an expiration date of April 2019. 2. The center owner confirmed that the Epipen was expired.

Plan of Correction: The center responded with the following: New medication had been requested from the parent before the expiration date. The parent brought in the new medication and the expiration date on the box was dated March 2020. It was discovered at the inspection that the Epipen canister was the expired medication and had been placed in the new box. The expired medication will be returned to the parent and a new one will be requested. Going forward, the center will check both the canisters of medication and the boxes upon receipt from the parent to ensure medication is not expired.
