An unannounced monitoring inspection was conducted from 4:45 - 5:45 pm. There were 22 children, ranging in age from six years to ten years of age, present with two staff supervising. The children were observed participating in a variety of center activities including legos, dolls, games, cards and completing homework. Five children's records were reviewed. Two onsite staff records were reviewed and in compliance. The last emergency escape drill was conducted 5/3/19 and the last shelter-in-place drill was conducted 3/19/19. There were no medications to review. Required information was posted. Standard #: 22VAC40-185-140-A Description: Based on a review of five children's files, the center did not obtain documentation that each child has received a physical examination before the child's first attendance or within one month after attendance. Evidence: The file for Child #5 (date of first attendance: 9/5/17) did not have documentation of a physical examination. Plan of Correction: Spoke with parent during inspection - physical documentation was sent to YMCA office previously. All will ensure file is updated with a copy of physical.
Standard #: 22VAC40-185-60-A Description: Based on a review of five children's files, it was determined that not all files were complete. Evidence: The file for Child #2 (date of first attendance: 9/5/17 ) did not have a street address or phone number for one emergency contact, or a street address for the second emergency contact. Plan of Correction: Will obtain missing information from parent.
