Standard #: 22VAC40-185-160-C Description: Based on record review, staff failed to resubmit tuberculosis test results every two years. Evidence: The tuberculosis results of Staff B was last submitted 05/04/2019. Plan of Correction: I will tell her to renew her tuberculosis screening.
Standard #: 22VAC40-185-70-A Description: Based on record review, staff records were incomplete. Evidence: 1. 1 of 5 staff records reviewed were incomplete. 2. Staff A, a lead teacher was missing a diploma or transcript. Plan of Correction: I will ask staff for that today.
Standard #: 22VAC40-185-240-C Description: Based on record review, staff working directly with children have not completed 16 hours of annual training. Evidence: Staff C had not completed 16 hours of annual training. Plan of Correction: I'll reach out to all substitutes and tell them about the requirements.
Standard #: 22VAC40-185-260-A Description: Based on observation, the center did not have a current annual fire inspection. Evidence: The last fire inspection was conducted 05/01/2018. Plan of Correction: It was paid for on April 11th. Usually they show up. I will follow up for an appointment.
Standard #: 22VAC40-185-290-3 Description: Based on observation, an electrical outlet was not covered with a protective cover. Evidence: An electrical outlet located on the wall in Room 204 was not covered with a protective cover. Plan of Correction: Corrected during inspection.
Standard #: 22VAC40-185-510-A Description: Based on observation, prescription and nonprescription medication did not have a written authorization form. Evidence: A prescription medication and nonprescription medication for Child A did not have their own written authorization form. Plan of Correction: I can ask them to do that today.
Standard #: 63.2(17)-1720.1-B-3 Description: Based on interview and record review, a complete out of state central registry search wasn't completed for staff living out of the state within the last 5 years. Evidence: Staff C has lived out of state within the last 5 years and an out of state central registry search was not complete. Plan of Correction: I sent it in and didn't receive a response. I'll try again.
