Comments:
An unannounced renewal inspection was conducted today, 5/7/19. A review of staff records took place at the main office on 5/10/19. Upon arrival to the center the children were preparing to go outside to play on the playground. A review of children's records and center posting's was also conducted. There are 19 children in care with four staff present. Lunch preparation was also observed. Should you have any questions please do not hesitate to contact me. Inspection at center began: 9:45 am ended 11:15 am. Record review began: 10:00 am ended 12:30 pm.

Violations:
Standard #: 22VAC40-185-330-B
Description: Based on observation during the center did not ensure that adequate resilient surfacing is under fall zones. Evidence: 1. The mulch under the lower slide measures 2-3 inches in depth.

Plan of Correction: The staff will ensure that the mulch is spread out and more added if needed to meet the six inch requirement.
