Technical Assistance:
Tecnical assistance was offered for classroom furniture, staff records and TB for new staff.

Comments:
An unannounced monitoring inspection was conducted from 10:45 am-2:15pm. 73 children were in care with 11 staff members present. Children were observed sorting colors using manipulatives, drawing free art, playing with legos and eating lunch. The lunch menu included turkey slices, whole wheat buns, broccoli and milk. 6 staff and 5 children's records, liability insurance documents, emergency drill records and attendance records were reviewed. Violations:
Standard #: 22VAC40-185-140-A
Description: Based on record review, the center did not obtain the report of a physical exam for a child within 30 days of enrollment. Evidence: Child # 1's first day of enrollment was 4/1/19. There was no documentation of a physical exam in Child # 1's record. The physical was not obtained by the center.

Plan of Correction: I will ask the parents to bring in a copy of the physical exam.

Standard #: 22VAC40-185-160-A
Description: Based staff interview, the center did not obtain a negative tuberculosis (TB) screening for a volunteer within 21 days of volunteering. Evidence: Staff # 1 stated that Staff # 2 started volunteering at the center on 4/1/19. A negative TB screening was not obtained for Staff # 2 with 21 days.

Plan of Correction: I will ask Staff to get TB screening by next week.

Standard #: 22VAC40-185-500-A
Description: Based on observation, children's hands were not washed with soap and running water before and after eating meals or snacks. Evidence: 1. In the infant classroom, 5 children were observed transitioning from play on the floor mat in the classroom to feeding chairs. 2. Two of the five children were observed eating lunch using their own hands. None of the children's hands were washed before lunch.

Plan of Correction: Staff will be retrained to make sure that proper hand washing is practiced.

Standard #: 63.2(17)-1720.1-B-2
Description: Based on record review and staff interview, the center did not obtain fingerprint results for an employee. Evidence: Staff # 3's first day of employment was 5/31/17. Staff # 1 stated that Staff # 3 informed her that she had done fingerprinting in 2018. The results were not available at the center for review by the inspector.

Plan of Correction: We will find the results and follow up with the inspector. If the results are not found, we will have the staff member redo them.
