Comments:
An unannounced complaint allegation investigation was conducted on 05/07/2019 concerning building maintenance. An inspection of the entire physical plant and interviews with the Director and the Assistant Director were conducted. Based on the preponderance of evidence the allegation was unfounded. Description: Based on observation, the center failed to ensure that hazardous substances, such as cleaning materials were kept in a locked place using a safe locking method that prevents access by children. Evidence: 1) The kitchen was unlocked and cleaning materials were stored inside accessible to children. 2) The utility closet was unlocked and cleaning materials were stored inside and accessible to children. 3) The hallway closet on the first floor, was unlocked and cleaning materials were stored inside and accessible to children.

Plan of Correction: 1) A meeting took place wit the Kitchen Manager and the Administration team. We discussed and agreed to keep the kitchen door locked at all times. 2) A sign has been posted to ensure the door will remain locked at all times. Administration checks the door periodically as a back up to having the sign posted. 3) Hallway closet has a working lock on the door and will remain locked at all times.
