An unannounced Monitoring inspection was conducted in the center on May 15, 2019. There were 15 children present, their ages ranged from 3 to 5 years. Two staff were actively supervising the children's activities. Two additional support staff were also present in the center. The children were observed during story time, circle time and individually selected centers. The children were observed at lunch time and during hand washing procedures. Staff were observed interacting with the children. A sample of 4 children's records was reviewed. A prescribed medication was observed in the center. Staff have completed medication administration training (MAT). Documentaiton of the center's annual fire and health inspections was current. The Licensing Inspector had a discussion with the program's Compliance Supervisor regarding children whom transfer from one center to another. The record shall be updated with current information and a new agreement signed between the attending center and the parent of the child. The inspection was conducted from 9:30 to 11:10AM. Standard #: 22VAC40-185-60-A Description: Based on a sample review of 4 children's records, the program failed to ensure the following information was complete in each child's record: 13.) previous child day care and 16.) first and last dates of attendance Evidence: A sample of four children's records was reviewed, information addressing previous child day care noted "none attended", the child had transferred to this center from another licensed Head Start program. The first date of attendance in this center was not documented. Plan of Correction: Staff will document current information in the child's record whenever a child transfers from one program to another program.
