An unannounced monitoring inspection was completed on 05/16/2019 from 2:40 P.M. until 6:15 P.M. At the time of the inspection 20 children were in care with two staff present. Five children's records, two staff records, required posted information, evacuation drills, first aid kits and supplies, fire inspection, emergency plan, health inspection, and outside play area were reviewed. Children were observed during arrival from school, free play, handwashing, snacktime, homework time, storytime, free choice activities, outside play time, and departure. Standard #: 22VAC40-185-500-A
Description: Based on observation, the center failed to ensure that that children washed their hands with soap and running water or a disposable wipe after eating snack. Evidence: 1. After the children finished cleaning up snack, ten out of eighteen children did not wash their hands and went directly to the door to line up for outside play time. 2. Staff verified that the children did not wash their hands after eating snack and went directly to line up for outside play time.

Plan of Correction: The site coordinator will start a practice of handing out disposable wipes after snack before dismissing the children from tables for clean up.

Description: Based on review of non-medical emergency supplies, the center failed to ensure that all non-medical emergency supplies were available on site. Evidence: 1. There was not a working, battery operated radio on site. 2. Staff verified that there was not a working, battery operated radio on site.

Plan of Correction: The site coordinator had the wrong size batteries available. The site coordinator will pick up the correct size batteries to keep on hand at the center.

Standard #: 22VAC40-185-550-B
Description: Based on review of the emergency plan, the center failed to contain all required procedural components in the emergency plan. Evidence: 1. The required components for staff training, and frequency of shelter-in-place drills was not in the emergency plan. 2. The emergency plan stated that the emergency plan would be updated and reviewed every September. The last documented day of emergency plan being revised and updated was May 2017.

Plan of Correction: The site coordinator will make the Executive Director and Board Members aware of the items that need to be included.

Standard #: 22VAC40-185-550-C
Description: Based on observation, the center failed to ensure that emergency maps contained shelter-in-place procedures/maps. Evidence: 1. The emergency map in the Clubhouse did not contain a shelter-in-place location. 2. The Site Supervisor verified that the posted emergency maps did not contain information regarding shelter-in-place.

Plan of Correction: The site supervisor will review the emergency maps and include the shelter-in-place location.
