Comments:
Thank you for your time and assistance with the unannounced renewal inspection conducted on May 20, 2019. The on-site inspection was held from 10:00a.m. to 1:15p.m. and staff records were reviewed at the Piedmont Family YMCA Administration Office from 2:00p.m. to 3:00p.m. On this date, there were 58 children present in six classrooms and in the care of 14 staff members. Two additional staff members were available to assist where needed. The total enrollment for the program was 83 children. All six classrooms were toured and observations were made in three of those rooms. There were six staff members' records and six children's records reviewed. Eight staff members and four children were interviewed. There was no medication on-site or being administrated. Lunch was observed to meet USDA guidelines. An exit interview was conducted on this date. The themes for the week included Mothers, People, and Animals and How Things Grow. Some items observed and reviewed included: injury reports, fist-aid and emergency supplies, required posted information, documentation of evacuation, shelter-in-place, and lockdown drills, warm water temperature, sunscreens and parent authorizations, updated emergency preparedness plan, annual health and fire inspections, playground, outdoor play, circle time, story time, infant care and feeding, diaper changing procedures, hand washing and bathroom procedures, lunch and nap time. Standard #: 22VAC40-185-500-A
Description: Based on observation and discussion with staff, one staff member failed to wash her hands after coming in contact with body fluid. Evidence: 1. Staff Member A in Classroom C wiped a child's nose with a tissue, threw the tissue in the trash and preceded to change the diaper of another child without washing her hands. 2. Staff Member B stated that Staff Member A had been trained on hand washing.

Standard #: 22VAC40-185-500-B
Description: Based on observation and discussion with a staff member, one staff member failed to use the foot-operated disposal system to dispose of the diaper and failed to clean the diaper changing pad. Evidence: 1. After diapering a child in Classroom C, Staff Member A proceeded to dispose of the diaper by using her hand to open the lid of the diaper bin. 2. After diapering a child in Classroom C, Staff Member A sprayed the changing pad with a bleach and water solution without first cleaning the pad with a soap and water solution. 3. Staff Member B stated that Staff Member A had been trained on diapering procedures.
