Comments:
A Subsidy Health and Safety inspection was conducted on May 22, 2019 to review supplemental health and safety requirements. This on-site visit was conducted from 11:00 a.m through 2:40 p.m. There were ninety-seven students in the care of fifteen staff. The medication and medication paperwork was reviewed for two children. Nine children's and nine staff records were reviewed and updated. A licensing inspection was also conducted this date; the two inspections combined cover all subsidy health and safety requirements. Only subsidy requirements that are not covered in licensing standards are reviewed during this supplemental inspection. Violations were found during the supplemental subsidy health and safety inspection and are identified on the violation notice. Areas observed were: outside play, free choice activity, hand washing/bathroom and diapering procedures, lunch and nap. Interactions between the children and staff were observed as appropriate and the children seem comfortable with the daily routine and schedule. Description: Based on record review, the center failed to train one volunteer on emergency evacuation, relocation, shelter-in-place and lock-down procedures. Evidence: 1. Based on record review, there was no documentation of one volunteer receiving training on emergency evacuation, relocation, shelter-in-place and lock-down procedures.

Plan of Correction: The volunteer will be trained on all emergency procedures.
