Comments:
An unannounced monitoring inspection was conducted from 10:30 a.m. - 1:30 p.m. There were 121 children in care being supervised by 19 staff members. The children were observed participating in structured center play, outdoor play, and preparing for lunch. Lunch served today is turkey and cheese sandwich, peas and carrots, peaches, and milk. Ten children's records were reviewed and in compliance. Seven staff records were reviewed. Eight medications were reviewed and in compliance. The most recent emergency escape drill was conducted 04/02/2019 and the most recent emergency escape drill was conducted 12/20/2018. Standard #: 22VAC40-185-160-A
Description: Based on review of seven staff records it was determined that not all staff records contained documentation of a TB (tuberculosis) screening within the first 21 days of employment. Evidence: Staff A (start date:11/26/2018) and Staff B (start date: 04/09/2019) did not have documentation of a TB screening on file.

Plan of Correction: Administration requested documentation of a TB screening for the staff members of Staff A and B.

Standard #: 22VAC40-185-500-A
Description: Based on observation of the two year old classroom it was determined that not all procedures for hand washing were followed. Evidence: Staff A was observed changing two children's diapers. While diapering both children staff A did not wash the child's hands after diapering. Staff A did not wash their own hands after diapering.

Plan of Correction: Administration re-trained Staff A on the proper hand washing procedures during the inspection.

Standard #: 22VAC40-185-500-B
Description: Based on observation of diapering in the two year old classroom it was determined that not all procedures for diapering were followed: 1. Staff A was observed diapering two children. Staff A failed to clean and sanitize the changing table after each diaper change. 2. The changing pad in the infant room had multiple cracks on top leaving the changing pad absorbent.

Plan of Correction: Administration re-trained Staff A on proper diapering procedures during the inspection.
