An unannounced monitoring inspection was conducted on May 28, 2019 from approximately 10:50 am to 1:55 pm. Five staff were present with fifteen children at the onset of inspection. Additional staff arrived during inspection. Five children's records and five staff records were reviewed.

Standard #: 22VAC40-185-130-A Description: Based on review, facility did not ensure 2 of 5 children's records have an immunization record prior to child beginning attendance. Evidence: The records for child #3 (start date July 30, 2018) and child #5 (start date February 19, 2019) did not have an immunization record. Plan of Correction: Staff will request information from parent of #3 today and request to have the immunization record by tomorrow. Parent of #5 was contacted during inspection and received the immunization. Information was put in child's record.
Standard #: 22VAC40-185-140-A Description: Based on review, facility did not ensure 3 of 5 children's records have a physical examination within a month of beginning attendance. Evidence: The records for child #3 (start date July 30, 2018), child #4 (start date April 9, 2019 and child #5 (start date February 19, 2019) did not have a physical. Plan of Correction: Parent of #5 was contacted during inspection and received the physical. Information was put in child's record. Parents will be contacted for #3 and #4 and the information will be in by tomorrow.
Standard #: 22VAC40-185-60-A Description: Based on review, facility did not ensure 1 of 5 children's records has documentation of required information. Evidence: The record for child #2 (start date May 2, 2019 did not have documentation parent's employment phone numbers. Plan of Correction: Staff will receive that information from parents by end of pick up today.
Standard #: 22VAC40-185-260-A Description: Based on review, facility did not ensure there is an annual fire inspection report from the appropriate fire official having jurisdiction. Evidence: The most recent fire inspection report available for review was completed on February 23, 2018 which is more than a year old. Plan of Correction: Staff will locate the results of the up-to-date inspection.
Standard #: 22VAC40-185-280-B Description: Based on observation, facility did not ensure hazardous substances such as cleaning materials, insecticides, and pesticides shall be kept in a locked place using a safe locking method that prevents access by children. Evidence: Two containers of bleach and one container of a disinfectant cleaner were observed on the floor in the kitchen which is adjoining to the toddler classroom. The door leading into the kitchen was not closed and locked. The containers were labeled "Danger...Keep out of Reach of Children". Plan of Correction: The kitchen door will be locked at all times.
