Evidence: 1. It was reported to the Inspector that "tummy time" is given to all infants who require it, for at least 30 minutes daily. 2. There were eleven days between 02/04/2019-03/21/2019 that there was no documentation of tummy time for a three month old child. 3. There were nine days between 02/04/2019-03/21/2019 that there was only 10 minutes of documented tummy time for a three month old child. 4. There were two days between 02/04/2019-03/21/2019 that there was only 15 minutes of documented tummy time for a three month old child. 5. There were two days between 02/04/2019-03/21/2019 that there was only 20 minutes of documented tummy time for a three month old child.

Plan of Correction: Administration will review with all infant staff the requirement for at least 30 minutes of documented tummy time daily for infants who cannot turn themselves over.

Standard #: 22VAC40-185-570-G

Administration verified that there had been formula put in the microwave to heat for the infants.
