Comments:
A Subsidy Health and Safety inspections was conducted on 05/29/2019 from 12:30 P.M. to 2:15 P.M. on site and from 2:30 P.M.-3:45 P.M. off site to review supplemental health and safety requirements. At the time of the inspection 11 children were in care with two staff and one site manager. Five children records, three staff records, one medication and required documentation, emergency evacuations, first aid kits, injury prevention plan, playground safety plan, health inspection, fire inspection, emergency plan, and posted required information was reviewed. Children were observed during television time, free choice activies, gross motor time in the gym, handwashing, and behavioral guidance. A licensing inspection was conducted on the same date as the SHSI supplemental inspection; the two inspections combined cover all subsidy health and safety requirements. Only subsidy requirements that are not covered in licensing standards are reviewed during this supplemental inspection. Exceptions to compliance are noted on the violation notice. Description: Based on documentation and interview, the vendor did not ensure to inform the department's representative as soon as practicable, but not more than two business days, of any injury to a child while under the vendor's supervision. When a referral is made for treatment from a medical professional. Evidence: The centers representative made contact with the department regarding a serious injury of a child on 05/03/2019, the injury occurred on 04/25/2019. All vendors are required to contact the department of any serious injury no more than two business days after the injury occurs.

Plan of Correction: In the future the administration will contact the department representative within two business days of a serious injury where a child seeks treatment from a medical professional.
