Comments:
An unannounced on site monitoring inspection was conducted during the program on May 16, 2019. There were 14 children present, their ages ranged from five to eleven years. As reported, there are 28 children currently enrolled. Two staff were assigned to supervise the children's activities. The children were observed during play in the school gym and outdoors. The children were served an afternoon snack. A sample of 5 children's records were reviewed. The emergency supplies were observed. As reported, no medications are being administered in the program. A discussion was held with the Site Director regarding active supervision of children and rotating among their activities. Three complete staff records were reviewed on May 22, 2019 at the central location. The inspection was conducted approximately from 3:40PM to 6:00PM. Standard #: 22VAC40-185-260-B
Description: Based on reviewed documentation, the annual approval from the health department for meeting requirements for water supply, sewage disposal, and food service had expired, the document was dated 02/08/18.

Plan of Correction: Staff will contact the local health department and will request an updated inspection report.

Standard #: 22VAC40-185-340-F
Description: Based on direct observations, staff failed to ensure children under 10 years was always within actual sight and sound supervision of staff during outdoor play. Evidence: Two staff and a group of children were observed during outdoor play. Children and one staff was located at the playground equipment which is located at the opposite end of the school entrance/parking area. Several children were observed at the swings, several children were at the steps leading from the parking lot down to the play area and a group of children was playing in the grassy area near the school building under a tree. The staff member was observed on the blacktop talking with a parent for over 10 minutes. The position of the staff and all the separate areas where the children were playing did not allow for sight/sound supervision of all children.

Plan of Correction: Staff will be reminded to follow center procedures and will rotate among the activities of the children.

Standard #: 22VAC40-185-540-C
Description: Based on inspection of the first ad kit supplies, the center failed to ensure the first aid kit was stocked with more than one triangular bandage.

Plan of Correction: Staff will add an additional triangular bandage to the first aid kit.

Standard #: 22VAC40-185-550-M
Description: Based on a sample review of written records of children's serious and minor injuries, the center failed to ensure each report contained at least two signatures, staff and parent signature or two staff signatures. Evidence: A written record documenting a minor injury for child #1 (12/4/18) was signed by only one staff.

Plan of Correction: Staff will monitor completed reports to ensure all information is completed.
