Comments:
An unannounced monitoring inspection was conducted from 9:10 am-1:15pm. At the time of entrance, 71 children were in care with 12 staff members present. Children were observed during a routine fire drill, snack time, engaged in group activities, singing, listening to the teacher read a book and outdoor play time. Interactions between the children and staff were positive . The site was organized and contained an abundant supply of developmentally appropriate materials. Part of the the center playground was under construction to install new playground equipment. A selection of staff and children records, the physical space, evacuation drills, attendance records, medications and the Emergency Preparedness Plan were reviewed. Violations:
Standard #: 22VAC40-185-270-A
Description: Based on observation, areas and equipment of the center inside were not maintained in a clean, safe and operable condition. Evidence:. 1. In the 2A class, the classroom rug and carpet stairs were stained or dirty. 2. In the 2B class, the children's kitchen play set had a hole near the play sink. The wooden table leg was loose. 3. In the School age classroom, the paint was chipping near the cubbies and under the TV wall. 4. In the Infant A classroom, the cabinet was coming off the hinge under the changing pad.

Plan of Correction: We will replace the the rug and stairs. We will tighten the table leg and will have maintance look to see if they can repair the sink. A repair order will be submitted for the school age room. The cabinet in the infant room was repaired during the inspection.

Standard #: 63.2(17)-1720.1-B-3
Description: Based on record review, the child day center did not obtain a copy of the results of a search of the central registry maintained by any other state in which the individual has resided in the preceding five years. Evidence: 1. Staff # 4 (Date of hire 3/17/2019) lived in New York in the past 5 years. The child care center had the central registry results for Virginia for Staff #4, but did not obtain the results of the central registry for New York. 2. Staff # 6 (Date of hire 1/31/2019) lived in Missouri in the past 5 years. The child care center had the central registry results for Virginia for Staff #4, but did not obtain the results of the central registry for Missouri.

Plan of Correction: I will follow up with the Out of State Background department or resend.
