Licensing Inspector arrived at 12:15 and departed at 1:30 PM. At the time of the inspection, there were 20 children (ages infant through preschool) in care with four staff. All children were observed sleeping. The sample size was nine staff and five children's records.

Standard #: 22VAC40-185-160-C Description: Based on review of nine staff records, it was determined that the facility did not ensure that at least every two years from the date of the last screening staff shall obtain and submit the results of a follow-up tuberculosis (Tb) screening. Evidence: The following staff had Tb screenings on file that were over two years old: - Staff #1 (last screening 12/12/16) - Staff #2 (last screening 4/11/16) - Staff #4 (last screening 12/12/16) Plan of Correction: Center director will ensure that all staff obtain updated Tb screenings and results are placed in records.
Standard #: 22VAC40-185-60-A Description: Based on review of five children's records, it was determined that the facility did not ensure that each child's record contained all the required elements. Evidence: 1. The record for child #1 (date of enrollment 5/17/19) did not contain the written agreements regarding emergency medical care, picking up ill children and notification of communicable disease in home. 2. The record for child #2 (date of enrollment 4/14/19) did not contain the work phone number and place of employment for the mother. 3. The record for child #3 (date of enrollment 5/22/18) did not contain the work phone number and place of employment for the mother or documentation of viewing proof of the child's identity and age. Plan of Correction: Center director will continue to review the records and contact the parents to obtain any missing information.
Standard #: 22VAC40-185-550-E Description: Based on review of the records at the facility, it was determined that the facility did not ensure that a record was maintained of the dates of practice emergency drills for one year. Evidence: Although center director stated that there were evacuation drills held in March and April, there was no documentation of these drills. Plan of Correction: Center director will ensure that all drills are documented when they are conducted.
Standard #: 22VAC40-185-560-F Description: Based on observation of posted documents, it was determined that the facility did not ensure that when centers choose to provide food, there shall be a menu listing foods to be served during the current one week period. Evidence: The menu that was posted was dated May 20 -24 and was not for the current week. Plan of Correction: The menu for the current week was posted during the inspection.
