A Subsidy Health and Safety inspection was conducted on 5/27/19 to review supplemental health and safety requirements. Licensing Inspector arrived at 12:15 and departed at 1:30 PM. At the time of the inspection, there were 20 children (ages infant through preschool) in care with four staff. All children were observed sleeping. The sample size was nine staff and five children's records. A licensing inspection was also conducted this date; the two inspections combined cover all subsidy health and safety requirements. Only subsidy requirements that are not covered in licensing standards are reviewed during this supplemental inspection. Violations were found during the supplemental subsidy health and safety inspection requirements and are identified on the violation notice.

Standard #: 22VAC40-665-510-B Description: Based on review of documents available at the center, it was determined that the vendor did not ensure that it maintained a written hard copy of attendance that documents the arrival and departure of each child as it occurs. Evidence: There was no attendance available for review. Plan of Correction: Vendor will ensure that attendance is taken on a daily basis and maintained.
Standard #: 22VAC40-665-580-E-1 Description: Based on review of nine staff records, it was determined that the vendor did not ensure that all staff had current certification in cardiopulmonary resuscitation (CPR). Evidence: The CPR certification for staff #1 expired 5/30/19. Plan of Correction: Center director will ensure that all staff maintain current CPR certification.
Standard #: 22VAC40-665-580-E-2 Description: Based on review of nine staff records, it was determined that the vendor did not ensure that all staff had current first aid certification. Evidence: The first aid certification for staff #1 expired 5/30/19. Plan of Correction: Center director will ensure that all staff maintain current first aid certification.
