Comments:
An unannounced monitoring inspection was conducted today. There were 75 children present with 18 staff, one behavior specialist and two administrators. Children and staff records were reviewed. Children were observed in teacher and self-directed activities including, play activities inside and outside, restroom routines, hand washing, lunch and nap. Additionally, required postings, annual inspections, medication, emergency evacuations, emergency items and first aid kit were reviewed. Violations regarding over the counter medications and meal service were observed and appear on the violation notice. These violations were reviewed with the administrators at the conclusion of the inspection.

Violations:
Standard #: 22VAC40-185-520-C
Description: Based on observation and interview the center failed to ensure that diaper cream is labeled with the child's name. Evidence: 1 - One diaper cream out of 15 topical preparations reviewed is not labeled with the child's name. 2 - Staff 1 confirmed that the diaper cream is not labeled with the child's name.

Plan of Correction: Corrected during the inspection. Staff will be reminded to make sure is diaper cream is labeled with the child's name when it comes into the classroom.

Standard #: 22VAC40-185-560-F
Description: Based on observation and interview the center failed to ensure that when food is supplied by the center the most recent, age-appropriate nutritional requirements of a recognized authority such as the Child and Adult Care Food Program of the United States Department of Agriculture (USDA) are followed. Evidence: 1 - The meal served to children today was sun butter and jam, fresh vegetables; cucumbers, celery sticks and carrot sticks, potato chips, whole wheat bread and milk. However, staff were unable to state how much vegetable and protein was served to each child during the meal. Therefore, there was no way to determine if USDA requirements were met during the lunch meal. 2 - Staff in the classrooms during lunch were unable to state how much protein and vegetable was served to each child.

Plan of Correction: Going forward center management will consult with the catering service to ensure that foods are prepared in such a way that when the staff are serving children they are aware of how much of each food group is being served to each child. This may mean cutting up fresh veggies into sizes that allow the food to be scooped with a measuring scoop.
