Comments:
An unannounced renewal inspection was completed on May 29, 2019 from approximately 10:45a.m. to 12:30p.m. Thirty-four children were in care and six staff were on premises during the inspection. Staff and children were observed in classrooms and on the playground engaged in activities to include outdoor free play, toileting, hand washing, lunch time and preparation for nap time. Interviews and interactions were conducted and medications were reviewed. Seven children?s records and six staff records were reviewed. The Owner/Director was available for the inspection and was present at the exit interview at which time inspection findings were reviewed and an Acknowledgement of Inspection form was signed and left with the licensee.

Description: Based on observation of the playground, the licensee failed to maintain resilient surfacing under equipment with moving parts or climbing apparatuses.
Evidence: 1. Several pieces of equipment to include a multicolored plastic slide, two swing sets, a yellow slide and a black climbing tire, located on the playground, approximately one to four feet tall at the highest points and surrounded by sand, lacked an adequate depth of sand under moving parts and climbing apparatuses. 2. The equipment required at least six inches of sand, but measurements taken ranged from approximately two to four inches of sand. 3. Administrator #3 acknowledged the resilient surfacing was not an adequate depth.

Plan of Correction: To ensure there is enough resilient surfacing (sand) underneath and surrounding the outside play equipment, additional sand will be raked or added to these areas to meet the depth requirement of 6?. This is scheduled to be completed the week of June 3-7, 2019.
