Technical Assistance:
Consultation provided on documentation on the injury/accident forms: making sure you are being descriptive of the type of injury sustained and documenting the specific time parents were told of injuries to their child. Please audit all children and staff files to make sure all of the required information is present and complete.

An unannounced monitoring inspection was conducted today with the director, who was present with 26 children with five staff providing supervision. The preschoolers were in the gym and the toddlers were outside playing at the beginning of the inspection. Upon their return to the classrooms the children washed their hands in preparation for lunch. Lunch consisted of meatballs with egg noddles, peas/carrots, pineapple and milk. Six children had 10 medications on site. There have been five injuries/accidents documented since 1/19. Six staff and five children's files were reviewed today. Please submit verification to your Licensing Inspector by 6/10/19 showing that you have sent off Fingerprint background checks for staff A and D. Arrival time:11:05 am Departure time: 2: 20 pm

Description: Based on a review of five children's files, it was determined that three children had incomplete emergency contact information. Evidence: There were no addresses or phone numbers for either of the two emergency contacts for child A ( enrolled on 10/31/18). There were no emergency contacts listed for child B ( enrolled on 2/4/19) There were no address for either of the two contacts for child C ( enrolled 8/27/18). There

Plan of Correction: We will audit the children's files and make sure all have the complete contact information listed.

Standard #: 22VAC40-185-280-B
Description: Based on observation, it was determined that in the 12 mos- 2year old classroom there was a spray bottle of orange sanitizing solution unlocked in the cabinet in the bathroom.

Plan of Correction: The spray was locked during the inspection.

Standard #: 22VAC40-185-510-C
Description: Based on a review of 10 medications for six children, it was determined that the parent authorization for medication for one child had not been renewed within 10 work days. Evidence: The parent authorization for the hydrocortisone for child A was dated 5/6/19. The authorization had expired and had not been renewed every 10 work days by the parent.

Plan of Correction: We will have the parent complete a new authorization form.

Standard #: 63.2(17)-1720.1-B-2
Description: Based on a review of six staff files and an interview with the director, it was determined that two staff did not have fingerprint background checks on file prior to employment. Evidence: Staff A was hired on 3/6/17 and does not have a fingerprint background check on file. Staff D was hired on 3/6/18 and does not have a fingerprint background check on file.

Plan of Correction: Neither staff will be allowed to return to the center until we have obtained their satisfactory fingerprint background check.

Standard #: 63.2(17)-1720.1-B-3
Description: Based on a review of six staff files and an interview with the director, it was determined that the center had not attempted to obtain out of state background checks for two staff within 30 days of their hire date. Evidence: Staff B ( hired 4/29/15) has lived in Maryland and Kansas over the last five years and the center has not attempted to obtain the out of state CPS background checks for either of those two states. Staff C ( hired 10/13/18) has lived in Pennsylvania over the last five years and the center has not attempted to obtain the out of state CPS background check for that state.

Plan of Correction: I printed off these forms and will have the staff complete them and I will send them off by the end of the week- 5/31/19.
